Cancellation Policy

Please note that the information provided in this form will be used solely for the purpose of DNA testing services by DNA On The Go! backed by Alliance DNA Laboratory. The deposit today is 100% non-refundable unless the DNA test is cancelled by DNA On The Go! or cancelled by an official party within 48 hours of the date of the test. Full payment is due on the day of the test. Only debit or credit card accepted on test day; no cash accepted. Mileage fees apply to any appointment location in excess of a 50-mile radius from our headquarters location. Any mileage fees associated with the test are due on test day. All mileage fees are subject to 62 cents per mile and is calculated for each stop from our headquarters address: 8651 Highway N ~ Suite 100 ~ Lake Saint Louis, MO 63367. Results are sent via email and postal mail within five (5) business days of the test being taken.
